Your Initial Visit

Your initial visit to our office will:
- Allow time to assess your concerns
- Involve a thorough medical and dental history
- Include a total mouth examination
Additional diagnostic material, such as X-rays, models of your mouth, or discussions with other doctors may be needed.

Once all the information is gathered to make a comprehensive plan, we will discuss your treatment options, the appointments required and the fees. We encourage you to ask questions regarding your mouth and the recommended treatment, and to bring a spouse or confidant along to listen to the consultation and make inquiries.  Dr. Gulbransen is a Delta Dental Premier Provider.  Please bring your insurance information with you on your first visit to our office.

Our friendly, caring staff will contribute to your overall comfort in our office. Your time is as valuable as our time, so we do our utmost to keep to scheduled appointment times. We look forward to conferring with you to help you to achieve optimal dental health.

 


What To Bring On Your First Appointment

For your convenience, go to the Online Forms tab to download health questionnaires and other forms you will need to fill out at your first visit. Our front office team is well versed in both dental and medical insurance issues.

We will assist you in maximizing your insurance benefits to the fullest. Please bring all of your insurance cards and any additional information, including referral cards and/or x-rays to your first appointment.
